If your insurance plan requires a referral — a formal piece of paper or number — please make sure to have the information before your visit. We can assist you in every that we can, but it is ultimately your responsibility to complete this necessary paperwork before you can be seen.

Accepted Insurance

Our office participates in most HMO, PPO and managed care plans and accepts most major insurances as well as standard Medicare. Your plan will be billed directly for covered services, but you will be required to pay a co-pay or deductible at the time of your visit. In the event that your health plan determines a service to be “non-covered” full payment is expected at time of service. Please bring your insurance card with you to each visit. Without your card, we will not be able to file your insurance claim in an accurate and timely manner.
